We are hiring for Advanced Practice Nurse/PMHNP positions across Northern NJ, Central NJ and Southern NJ.
Salary range: $130,000 to $145,000 a year.
Responsibilities- Conduct visits to patients in accordance with local, state, and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team.
- Facilitate access to appropriate support models of care, such as rehabilitation or ongoing aggressive treatment.
- Consult with other members of the interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts, and documents.
- Assist in developing clinical practice guidelines and standards to support quality care, as needed.
- Collaborate with patients, caregivers, medical directors, and members of the interdisciplinary team.
Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Professional Experience- License as a Nurse Practitioner in the state of operation.
- PMHNP certification.
- At least 1 year of experience in family, general practice, geriatric care, or a related field.
- Experience in nursing homes, hospitals, assisted living, or acute rehab settings is a plus.
- Commitment to ongoing professional development and clinical excellence.
- Valid driver’s license and reliable transportation.
- Willingness to travel locally.
