Responsibilities
  • Collects data from patient, family or significant others and records on Initial Data Base form.
  • Twenty-four hour nursing assessment and provides data to RN for MDS inclusion.
  • Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
  • Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, resident care, TPR) according to established standards.
  • Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
  • Documents pertinent information regarding patient responses and nursing interventions in patient’s medical record.
  • Administers medication according to hospital policies; understands and monitors age specific data including meds, lab/diagnostic results, etc.
  • Applies principles of infection control and safety in the performance of patient care.
  • Communicates effectively with charge nurse and other members of the health care team regarding patient’s needs and/or changes in condition.
  • Responds appropriately in crisis situations; able to prioritize.
  • Reports significant unit problems to the charge nurse.
  • Patient care responsibilities are delivered with knowledge of patient growth and development appropriate to the ages of the patients served and compliant with the Bill of Rights.
  • Demonstrates customer service attributes including respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience, and helpfulness; responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.
  • Maintains established departmental policies and procedures, quality assurance program, safety, environmental and infection control standards. Assesses gaps and helps address them as appropriate.
  • Understands and adheres to the Medical Center’s Code of Conduct.
  • Familiar with the Medical Center’s Mission, Vision, and Values Statements.
  • Orients patient and family to new environment; develops rapport with patient’s family and/or significant others.
Position Qualifications
  • Current NJ license as an LPN.
  • Successful completion of Progressive Care Orientation Program required.
  • American Heart Association Basic Life Support certification required.
  • IV certification preferred.
  • Clinical nursing skills appropriate for an LPN.
  • Good oral and written communication skills.
  • Good interpersonal skills.
  • Speaks, reads and writes English to the extent required by the position.
Education

Graduation from an accredited school of practical nursing or equivalent education as described by the state education law is required.

Job Setting/Physical Demands
  • All patient care areas; contact with patients, visitors, staff.
  • Frequent prolonged standing/walking.
  • Lifts, positions, pushes, and/or transfers patients.
  • Protective clothing worn: face shield, gloves, impermeable gown, particulate respirator.

Salary: Salary commensurate with experience within posted range.
