Role OverviewThe Registered Nurse Non‑Acute functions as a caregiver for a specific patient population to achieve desired outcomes. The position expands knowledge and skills, delivers effective direct care as part of an interdisciplinary team, and directs and oversees care provided by other caregivers through delegation, validation, evaluation, and coordination.
ResponsibilitiesA day in the life of a Registered Nurse Non‑Acute involves delivering nursing practices according to the most up‑to‑date evidence‑based research, nursing policies, and standards of care to provide safe, individualized patient care.
- Collect comprehensive data pertinent to the patient’s health or situation.
- Perform age‑ and disease‑specific assessment and data collection independently in a systematic manner focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes.
- Identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulate age‑ and disease‑specific patient/family‑centered goals and outcomes based on assessment data.
- Develop and coordinate a patient‑ and family‑focused, prioritized plan encompassing the continuum of care.
- Communicate, coordinate, and implement the plan of care.
- Provide ongoing communication with patient/family, nursing colleagues, and other health care team members regarding status, rendered care, and treatment plans.
- Implement age‑ and disease‑specific care in an organized and timely manner.
- Provide or coordinate patient/family education as needed and ensure understanding.
- Act according to established protocols in clinical situations, including communication and documentation.
- Provide nursing care and treatments that reflect patient needs, advocate appropriately, and demonstrate evidence‑based nursing practice and compliance with standards.
- Perform continuous point‑of‑care electronic documentation of assessments, plans, interventions, and evaluations of rendered care.
- Administer medication utilizing bar‑scanning and documentation technologies to ensure safe delivery.
- Participate in multidisciplinary discharge planning to ensure safe transition and continuum of care.
- Evaluate progress toward attainment of outcomes.
- Utilize critical thinking skills based on nursing knowledge to achieve desired outcomes.
- Initiate interventions related to patient’s change in condition and unexpected response to care.
- Identify ethical issues in the practice setting and bring them to the attention of team members.
- Recognize and respond to opportunities to enhance patient satisfaction.
- Acquire and maintain competencies, certifications, and continuing education to meet facility accreditation or licensing requirements.
- Adhere to the American Nurses Association Code for Nurses.
- Lift a minimum of 35 lbs., push or pull a minimum of 10 lbs., and stand a minimum of 6 hours a day.
- Adhere to HMH’s organizational and managerial competencies and standards of behavior.
- Perform other duties and/or projects as assigned.
- BSN in progress and/or willing to acquire within three (3) years of hire.
- Ability to learn quickly and adapt to changing patient needs.
- Strong sense of accountability, teamwork, dedication to ongoing education, and passion for delivering high quality care with a patient‑focused approach.
- National Certification in area of specialty when eligible.
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support (HCP) Certification within 60 days of entering position.
