LICENSED PRACTICAL NURSE (LPN) - BEHAVIORAL HEALTH SERVICES - ACUTE GERI UNIT (C-G)- FULL TIME EVENING (20860)

We are dedicated to providing high-quality, compassionate care to our diverse community. The Bergen New Bridge Medical Center welcomes a Licensed Practical Nurse to work in Behavioral Health Services on our Acute Geropsy Unit during evening shifts.

Responsibilities
  • Collect and record patient data from patients, families, or significant others on Initial Data Base forms.
  • Conduct 24‑hour nursing assessments and provide data to the RN for MDS inclusion.
  • Contribute to the formulation and updating of the plan of care and implement it under the direction of the Registered Nurse.
  • Deliver direct nursing care to assigned patients, including medications, treatments, dressing changes, resident care, and TPR per established standards.
  • Recognize changes in patient condition, respond appropriately, and notify charge nurse as needed.
  • Document pertinent information regarding patient responses and nursing interventions in the medical record.
  • Administer medication according to hospital policies; monitor age‑specific data including medications, lab/diagnostic results, etc.
  • Apply principles of infection control and safety in all patient care activities.
  • Communicate effectively with the charge nurse and other team members about patient needs and changes in condition.
  • Respond appropriately in crisis situations and prioritize care.
  • Report significant unit problems to the charge nurse.
  • Deliver patient care in accordance with the Bill of Rights and appropriate for the ages of patients served.
  • Maintain a professional, respectful, and helpful attitude in line with Customer Service Performance expectations.
  • Adhere to departmental policies, quality assurance, safety, environmental, and infection control standards; identify and address gaps in policies and procedures.
  • Understand and comply with the Medical Center’s Code of Conduct, Mission, Vision, and Values statements.
  • Orient patients, families, LPNs, nurses’ aides, and orderlies as required.
  • Utilize the hospital computer system effectively in accordance with hospital policy.
  • Participate in unit committees, continuous quality improvement activities, patient care conferences, staff development activities, and other assigned duties.
Position Qualifications
  • Current New Jersey license as an LPN.
  • Successful completion of the Progressive Care Orientation Program.
  • American Heart Association Basic Life Support certification required.
  • IV certification preferred.
  • Clinical nursing skills appropriate for an LPN.
  • Good oral and written communication skills.
  • Good interpersonal skills.
  • Proficient in speaking, reading, and writing English.
Education

Graduation from an accredited school of practical nursing or equivalent education as required by state law.

Job Setting / Physical Demands
  • All patient care areas; contact with patients, visitors, and staff.
  • Frequent prolonged standing and walking.
  • Lifting, positioning, pushing, and/or transferring patients.
  • Use of protective clothing: face shield, gloves, impermeable gown, particulate respirator.
