Job Duties

Patient Care

  • Collects data from patient, family or significant others and records on Initial Data Base form.
  • Twenty-four-hour nursing assessment and provides data to RN for MDS inclusion.
  • Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
  • Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, resident care, TPR) according to established standards.
  • Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
  • Documents pertinent information regarding patient’s responses and nursing interventions in patient’s medical record.
  • Administers medication according to hospital policies; understands and monitors age specific data including meds, lab/diagnostic results, etc.
  • Applies principles of infection control and safety in the performance of patient care.
  • Communicates effectively with charge nurse and other members of the health care team regarding patient’s needs or changes in condition.
  • Responds appropriately in crisis situations; able to prioritize.
  • Reports significant unit problems to the charge nurse.
  • Demonstrates understanding and complies with all regulatory agencies; ability to assume charge in the absence of a charge nurse.
  • Performs patient teaching and nursing research activities (CQI).
  • Maintains established departmental policies and procedures, quality assurance program, safety, environmental and infection control standards. Assesses gaps and creates necessary policies.
  • Understands and adheres to the Medical Center’s Code of Conduct, Mission, Vision and Values Statements.
  • Other duties: orienting patients and families, orienting LPNs and nurses’ aides, using the hospital computer system, participating in unit committees, CQI activities, patient care conferences, staff development, assisting personnel, performing other duties as assigned.
Position Qualifications
  • Current NJ licensed Licensed Practical Nurse license.
  • Current American Heart Association Basic Life Support certification.
  • IV Certification.
  • Experience with Med pass, Encore, and Dressing change.
  • Successful completion of Progressive Care Orientation Program.
  • Clinical nursing skills appropriate for an LPN.
  • Good oral and written communication skills.
  • Good interpersonal skills.
  • Speaks, reads and writes English to the extent required by the position.
Education
  • Graduation from an accredited school of practical nursing or equivalent education as described by state law.
Job Setting / Physical Demands
  • All patient care areas; contact with patients, visitors, staff.
  • Frequent prolonged standing/walking.
  • Lifts, positions, pushes, and/or transfers patients.
  • Protective clothing worn: face shield, gloves, impermeable gown, particulate respirator.

Salary commensurate with experience within posted range.
