• Demonstrate flexibility with schedule and travel to support staffing needs across multiple communities.
• Oversee medication distribution as stated on physician orders, ensuring strict adherence to community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety.
• Upon thorough assessment, create personalized service plans tailored to individual care needs while championing resident independence and dignity.
• Cultivate communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well‑being of residents.
• Contribute toward quality assurance and performance improvement initiatives.
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.

• Current state licensure as a Licensed Practical Nurse (LPN).
• Must hold a valid driver's license as travel may be required.
• Current CPR certification.
