Horizon Health is a critical access, rural health facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including family practice and specialty clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 through community education, emergency services, and outpatient care. Position Summary

In accordance with department policy and the patient's plan of care, the RN provides direct and/or indirect nursing care, treatment, and instruction in the homes of patients referred by the Home Care department.

Position Requirements

Registered Nurse with current license in the State of Illinois Minimum one (1) year of relevant RN experience, preferred two (2) years acute/LTC experience

Reliable transportation with current Illinois driver's license and proof of auto insurance

Strong leadership, decision-making, problem solving and organizational skills

Must be willing to participate in quality improvement process

Graduated from an accredited school of Nursing Current CPR certification or within thirty (30) days of hire Ability to communicate well verbally and in writing

Knowledge of nursing process and use of the same Ability to perform complex and varied tasks in a fast-paced work environment subject to stress and interruption

Essential Functions (Responsibilities/Accountabilities)

Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various complex home situations

Participates in the on-call system as directed, providing availability of skilled nursing services at all times according to agency policy

Teaches and supervises family members, auxiliary nursing personnel, or others giving nursing care Coordinates with all services in the home Plans with the family and physician for services within the physical, financial, and emotional resources of the family; helps the family accept responsibility of providing care Gives prescribed treatments or emergency care, teaches, and supervises patients, family members or others who give such treatments

Initiates nursing measures to prevent complications or to minimize the disabling effects of disease or injury

Observes, evaluates, and reports to physician patient's physical and emotional condition, reaction to drugs or treatments, and changes in condition or needs

Performs diagnostic tests as authorized and obtains laboratory specimens when indicated

Completes accurate documentation in medical record of services provided in accordance with the plan of care Helps individuals and families to accept and adjust positively to physical, mental, and social limitations

Complies with policies and procedures of the organization/department

Participates in quality improvement activities

Makes necessary revisions in the Plan of Treatment Participate in in-service programs for agency staff

Provides comprehensive assessment, planning, implementation, and evaluation for home health patients as the primary nurse

Assesses physical, functional, psychosocial, and cognitive status of the home care patient

Assesses the home environment for safety, infection prevention, and community resource needs

Collects information from the patient, family, physician, and other community resources

Incorporate multidisciplinary data into the nursing assessment of the home care patient Assess the presence of advanced directives and facilitate further action in obtaining or implementing if indicated

Plans with the patient, family, and physician for care which is feasible within the physical, financial, and emotional resources of the family

Establish individualized, measurable goals in consultation with patient, family, and other health care providers

Maintain a working knowledge of community resources and refer patients appropriately Utilize home care standards, regulatory guidelines, and standards of care in developing an individualized care plan Maintains technical skills according to organization standards as measured by competency assessment during orientation and annually/ as needed Implements safe, competent care with home care patients and families with organization policies, procedures, and standards of care Accurately follows established infection prevention and safety policies and procedures

Directs the use of equipment and supplies in an efficient and cost-effective manner Coordinates and directs home care aide assignments, prepare written instructions, and supervises the aide in the home Coordinates and directs Licensed Practical Nurse activities, prepares care plan, and provides oversight and supervision of care and documentation
