Position: FAMILY NURSE PRACTITIONER EZ CARE

Salary Range: $90,000.00 To $ Annually

Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. Position Summary:

A Family Nurse Practitioner/Physician Assistant assesses, diagnoses, and treats patients on an acute, urgent care basis. This includes maintaining individual patient records, performing medical examinations and prescribing medications as well as dealing with acute or specialized medical issues.

Essential Functions:
  • Responsible for providing episodic care, as well as screening and educational assistance to the clinic’s patients
  • Recognize diversity of patient population and treat all warmly and kindly
  • Work in collaboration with other members of the healthcare team and corporate team to provide quality service to patients
  • Provide quality care to patients by following the most current evidence-based guideline and procedure of practice (USPSTF guidelines)
  • Following the clinic’s evidence-based guidelines, perform diagnostic screenings
  • Take part in quality assurance audits, including medication administration review, and peer review
  • Ensure company’s objectives regarding financials, patient engagement, and healthcare quality
  • Promote company’s services to patients and carry out promotional campaigns
  • Ensure participation in continuing education and training to keep abreast of current developments in the industry and as required for maintenance of state licensure
  • Ensure state licensure and national certification remain current and valid
  • Ensure continuous flow of communication with leadership of healthcare clinic to enhance quality of healthcare service to patients
  • Collaborate with the supervising physician as needed and as regulated by the state
  • Participate in quarterly meeting to discuss clinic operations
  • Perform other tasks that may be assigned by a superior, such as the chief nursing officer or the vice president.
  • Specialist referral coordination (orthopedic, physical therapy, dermatology, etc)
  • E‑prescribing with generic medications as a focus
  • Lab collection and processing
  • Minor procedures such as I&D, suturing of minor lacerations, splinting of extremities, ear wax removal.
  • School and sports physicals
Hours:

Full‑Time with varying schedule including some holidays and weekends, 66hrs per pay period

Position Requirements :
  • Possess current and valid license from the State Board of Illinois
  • Officially accredited to practice as a Family Nurse Practitioner (FNP) Or Physician Assistant (PA)
  • Meet all state requirements and obtain valid Prescriptive Authority
  • Meet and keep valid all state and national legal requirements to practice
  • Hold CPR (BLS and ACLS) certification
  • Competent in using electronic medical records and information systems in providing effective documentation of patient care
  • At least six months of clinical experience working as a FNP/PA
  • Experience working with pediatric patients
  • Ability to achieve high performance working with diverse groups of individuals and leadership in a team environment
  • Passionate about rendering quality and compassionate service to patients and growing the clinic
  • Possess exceptional organizational skills, multitasking ability, and effective communication skills to ensure patients are well informed, and the clinic is well promoted internally and to the public.
Pay Rate:

Pay ranges from $90,000 to $120,000/year (rate of pay is based on applicable years of experience)
