We are seeking a compassionate and reliable MDS Coordinator (LPN or RN) to join our care team.
Responsibilities- Attending weekly education meetings to stay updated on MDS changes
- Coordinating the Resident Assessment Instrument (RAI) process per state and federal guidelines
- Completing accurate MDS assessments and supporting documentation
- Evaluating residents' care plans, auditing medical records, and collaborating with the interdisciplinary team
- Performing additional tasks as assigned and maintaining confidentiality of protected health information
- Active LPN or RN license in Tennessee
- Minimum of 3 years nursing experience, including supervisory roles
- MDS training completed within 6 months of hire
- RAC Certification preferred
