, delivering skilled geriatric rehab services to residents in a structured, supportive environment.

As an Assisted Living PT
, you will:

• Perform comprehensive physical therapy evaluations in assisted living communities
• Develop and implement individualized Plans of Care (POC)
• Provide skilled geriatric physical therapy and assisted living rehab services
• Treat orthopedic, neurological, balance, and mobility impairments
• Deliver interventions focused on:
  • Fall prevention
  • Gait training
  • Strength and balance retraining
  • Functional mobility and ADL performance
• Educate residents and caregivers on:
  • Safe transfers
  • Assistive devices
  • Home exercise programs
• Document accurately and efficiently in EMR
• Collaborate with physicians, nursing staff, and interdisciplinary rehab teams
• Supervise PTAs when applicable
• Ensure compliance with assisted living and senior care regulations

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an APTA-accredited program
• Active Texas Physical Therapist license (or Compact eligibility)
• 1+ year experience in Assisted Living, SNF, Home Health, or Geriatric Rehab preferred
• CPR certification
• Strong clinical judgment and documentation skills
• Experience with senior care and assisted living regulations preferred
