PIVOT Rehabilitation is hiring a Licensed Home Health Physical Therapist Assistant (PTA) for a PRN home health physical therapy assistant job providing in‑home physical therapy services to adult and senior patients in Dallas, TX and surrounding DFW areas

• Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT)
• Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT
• Treat patients with orthopedic, neurological, cardiac, and post‑surgical conditions
• Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities
• Apply manual therapy techniques and non‑complex modalities including heat, cold, electrical stimulation, and other physician‑ordered treatments
• Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies
• Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist
• Complete accurate and timely electronic documentation in compliance with Medicare home health regulations
• Follow infection control standards and home health safety protocols
• Participate in QAPI and agency performance improvement initiatives

• Associate Degree from an accredited Physical Therapist Assistant (PTA) program
• Active Texas Physical Therapist Assistant (PTA) license
• Home health physical therapy experience preferred (1+ year preferred)
• Knowledge of Medicare home health documentation standards preferred
• Current CPR certification
• Strong communication, clinical judgment, and time‑management skills
• Valid Texas driver’s license
• Reliable transportation and current auto insurance
