Licensed Vocational Nurse; LVN Rehabilitation

The Licensed Vocational Nurse (LVN) will provide patient care by completing assessment and plan-of-care interventions, documenting care, and supporting patients with daily activities while collaborating with RNs and physicians.

• Contribute to the assessment of patients by collecting data for analysis by an RN.
• Perform plan-of-care interventions, including specimen collection and wound care; may administer medication.
• Document patient care provided.
• Assist RN with the development of plan of care.
• Assist patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation.
• Monitor, document, and communicate patient status and responses to interventions with RN, physician, patient, and family.
• Advocate for the physical and emotional well‑being of the patient.
• Participate in performance improvement and continuing education.
• Provide quality patient care and work flexible shifts/units as needed.
• Maintain regular and reliable attendance.
• Perform other duties as assigned.

• Current state LVN license and an associate’s degree from an accredited nursing school.
• Basic Life Support certification required within 30 days of hire.
• ACLS and PALS certification required within six months of hire.
• One year of experience preferred.
• ASLS certification preferred.
