Home Care Nurse - RN - LPN

Shift Availability

Park City:
Every day of the week, 12 hour shifts days and nights, 7-7

• Medication set up and oversight
• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in‑home assessments
• Monitor and track changes in the client's conditions and needs, and keep the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Weekdays include going to school with client
• Total personal cares

• Bonus for hours worked

• Joint Commission accredited

• Must have at least 1 year of nursing experience
• Current unrestricted RN or LPN license in Utah
• Certified CPR
• Negative TB skin test or chest X‑ray
• Must pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull, and lift 75 pounds
