Home Care Nurse - RN - LPN

Client Overview

Client is a 19‑year old male with TBI. He is wheelchair‑bound, has limited speech, hemiplegic, and prefers a nurse with neurological and/or behavioral health experience.

• Park City:
  Night shifts from 7pm to 7am, 2 or 3 nights per week

• Medication set up and oversight
• Provide thoughtful care to clients based on a personalized Plan of Care
• Provide in‑home assessments
• Monitor and track changes in the client's conditions and needs, and keep the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Perform total personal cares

• Must have at least 1 year of nursing experience
• Current unrestricted RN or LPN license in Utah
• Certified CPR certification
• Negative TB skin test or chest X‑ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull
• Must be able to lift 75 pounds
