Overview
This recruitment is only open to current Colorado residents. This posting may be used to fill multiple vacancies full time. Colorado Mental Health Hospital in Pueblo (CMHHIP) is a 24 hour behavioral health acute care facility providing evaluation and treatment for patients with psychiatric and medical issues. The program focuses on inpatient psychiatric evaluations and treatment for severely emotionally disturbed and/or cognitively impaired older adults, with the goal of returning patients to their community.
Direct treatment includes individual, family, group work and psycho-educational treatment.
You are required to work at your assigned work location during your scheduled work times.
Monday - Friday 8:00 a.m.
- 5:00 p.m. at the CMHHIP Campus. Work arrangements are subject to change at any time.
This position exists to provide therapeutic interventions including psychosocial therapies, individual, family and group treatment to severely emotionally disturbed/cognitively impaired patients and their families. This position completes assessments and disposition planning, interagency communication with residential care facilities, social services, community mental centers, etc., to ensure continued appropriate level services following discharge. Individualizes psychosocial therapy approaches to meet the specific needs of the patient population.
Provide training and consultation to other professional staff.
Duties include:
- Assessments: Complete a comprehensive biopsychosocial assessment of patients being admitted for inpatient psychiatric treatment; interview the patient; obtain collateral information; complete CCAR and Bio-Psychosocial assessment; re-assessments and progress documentation; inform team of assessment information.
- Community Reintegration: Coordinate discharge planning; act as liaison between hospital and community agencies; provide case management and referrals; attend meetings with agencies.
- Treatment Planning: Participate in treatment plan direction and review; design interventions with the multidisciplinary team; provide required documentation.
- Psychosocial Education: Assess education needs; provide psycho/social education; educate patients and families about symptoms, management, relapse prevention, and level of care; document entries.
- Treatment: Conduct individual, group, and family therapy; interpret treatment issues; assess and document effectiveness; guide milieu staff on interventions and homework.
- Professional Development: Participate in orientation, workshops, supervision, training; provide consultation and training to staff.
- Other Duties: Perform other duties as assigned.
Current, valid Colorado or State licensure participating in the Social Work Licensure Compact (as applicable):
- Licensed Social Worker (LSW) OR
- Licensed Clinical Social Worker (LCSW) OR
- Licensed Professional Counselor (LPC) OR
- Licensed Marriage and Family Therapist (LMFT) OR
- Licensed Addiction Counselor (LAC)
AND one (1) year of experience with current, valid licensure from a state participating in the Social Work Licensure Compact based on the work assigned to the position.
Preferred Qualifications- MSW in Social Work
- Knowledge of casework methods and their applications
- Knowledge of human growth and behavior, sociological and psychological treatment practices
- Knowledge of interviewing skills and techniques
- Ability to express ideas verbally and in writing
- Ability to exercise good judgment in evaluating situations
- Full background check and reference check required
- State residency at time of application and throughout employment
- Motor Vehicle Check if driving a state-owned vehicle
- Possess a valid Colorado driver’s license or equivalent for positions within 30 miles of Colorado border; out-of-state license holders must obtain Colorado license within 30 days of employment
- Other program-specific conditions of employment
- Disclose prior terminations or resignations in lieu of termination and provide prior employee number if applicable
- Respect patient boundaries; no sexual, social, personal or financial relationships with patients
- Complete CMHIP New Employee Orientation and related training (Defensive Tactics, CTI, CPR, etc.)
- Physical requirements may include lifting, restraining, and safety-related tasks; ability to observe and respond to safety concerns
- Ability to read and write in English; able to travel if required
- TB screening and annual influenza vaccination
