Licensed Practical Nurse

Life Care Centers seeks a Licensed Practical Nurse to provide compassionate, high quality care to senior residents in our rehabilitation and long-term care community. The LPN will administer medications and treatments, monitor resident conditions, document care, and support care plan implementation under RN and physician direction. You will collaborate with an interdisciplinary team, communicate with families, and help ensure regulatory and safety standards are met.

Responsibilities

• Provide direct nursing care to residents in a senior living/rehab setting
• Administer medications and treatments per provider orders
• Monitor, record, and report resident condition and vital signs
• Document care accurately in electronic medical records
• Support development and implementation of individualized care plans
• Collaborate with RNs, physicians, and therapy staff on patient care
• Educate residents and families on care plans and health needs
• Maintain infection control, safety, and regulatory compliance
• Assist with admissions, discharges, and care transitions
• Contribute to a patient-first, team-based care environment
