LVN ​/ LPN – LTC​/SNF

Long-Term Care/Skilled Nursing Facility Nurse

Provide skilled nursing care to residents in long-term care or skilled nursing facilities. Ensure patient comfort, safety, and adherence to care plans while maintaining compliance with facility policies.

Responsibilities:

• Administer medications and treatments accurately per physician orders.
• Monitor residents’ health status and report changes to the healthcare team.
• Document patient care activities diligently and in a timely manner.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Adhere to infection control and safety protocols at all times.
• Maintain professional relationships with residents and their families.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Must have recent experience in LTC or SNF settings.
• Demonstrate excellent communication and interpersonal skills.
• Ability to work effectively in a team environment.
• Fulfill all facility requirements, including vaccination documentation.
• Bring a valid  photo badge for facility access.
