Licensed Practical Nurse

Parkersburg Care Center seeks a Licensed Practical Nurse to provide compassionate, hands-on care for residents in our skilled nursing and rehabilitation facility. In this resident-focused role, you will administer medications, perform treatments and wound care, monitor vital signs, and document care using EMR systems. You'll collaborate with RNs, therapists, and physicians to support individualized care plans, rehabilitation goals, and quality-of-life outcomes.

Responsibilities

• Provide direct nursing care to residents, including treatments, wound care, and assistance with ADLs
• Administer medications and monitor for effectiveness and side effects
• Assess resident conditions, record vital signs, and report changes to the RN and provider
• Document care accurately in EMR and maintain up-to-date care plans
• Collaborate with RNs, therapists, and physicians on resident care and rehabilitation goals
• Educate residents and families on care plans and promote safety and comfort
• Follow infection control, fall prevention, and facility safety protocols
• Support a compassionate, resident-focused environment and mentor CNAs as needed
