LVN​/LPN LTC​/SNF

Job Summary:

We are seeking a qualified LVN / LPN to join our healthcare team for a dynamic short-term assignment. The ideal candidate will provide exceptional patient care in a long-term care or skilled nursing facility setting.

Responsibilities:

• Assess and monitor patient conditions to ensure optimal care delivery.
• Administer medications and treatments in accordance with physician orders.
• Document patient care activities accurately and timely.
• Communicate effectively with patients, families, and the healthcare team.
• Follow all facility protocols and safety regulations to maintain a secure environment.
• Maintain confidentiality of patient information at all times.

Qualifications:

• Valid LVN / LPN license in the state of practice.
• Proven reliability with a strong track record of consistent attendance.
• Ability to work three days one week, and four days the next, every other weekend.
• Capable of adhering to strict attendance policies with no more than two call outs and no NCNS.
• Must bring a valid state or federal-issued  wear a photo badge during shifts.
• Proof of COVID-19 vaccination or exemption documentation required.
