LVN​/LPN – LTC​/SNF

Job Summary:

Provides nursing care to residents in long-term care and skilled nursing facilities. Ensures effective patient management while adhering to safety and compliance standards.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor, assess, and document patient conditions and progress.
• Collaborate with healthcare team to develop and update individualized care plans.
• Ensure compliance with infection control and safety protocols, including COVID-19 vaccination requirements.
• Respond promptly to patient needs and emergency situations.
• Maintain a clean and safe environment, safeguarding resident well-being.

Qualifications:

• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) certification required.
• Proven reliability with experience in LTC/SNF settings preferred.
• Ability to work flexible schedules, including every other weekend.
• Strong communication skills and compassionate patient care approach.
• Must possess a valid state-issued  be able to wear a photo badge at all times.
• COVID-19 vaccination or approved exemption required, with additional doses obtained where mandated.
