LVN​/LPN – LTC​/SNF

Job Summary:

Seeking a dependable LVN / LPN for a short-term contract at a long-term care or skilled nursing facility. This role involves providing essential nursing care to residents and supporting the healthcare team.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor patient vital signs and report any changes promptly.
• Document resident care activities in compliance with facility policies.
• Collaborate with healthcare team to develop and update resident care plans.
• Assist residents with activities of daily living to promote comfort and safety.
• Follow infection control procedures, including COVID-19 vaccination and exemption documentation.

Qualifications:

• Hold a valid LVN or LPN license recognized in the state.
• Demonstrate proven reliability and dependability in previous roles.
• Possess excellent communication skills and compassionate bedside manner.
• Be able to provide a valid state or federal issued  proof of COVID-19 vaccination or exemption.
• Maintain a professional appearance with a photo badge at all times.
• Be prepared for potential contract completion bonus eligibility with consistent attendance.
