Registered Nurse (RN) – Wound Care Nurse
We are seeking an experienced and compassionate Wound Care RN to join our long‑term care team. The ideal candidate will be responsible for assessing, planning, implementing, and evaluating wound care treatment plans for residents with acute and chronic wounds. This clinical leader will work collaboratively with the interdisciplinary team to ensure the highest standard of care and regulatory compliance.
Responsibilities- Provide comprehensive wound assessments and ongoing monitoring for residents with pressure injuries, surgical wounds, diabetic ulcers, and other skin integrity concerns.
- Develop individualized wound care treatment plans according to evidence‑based best practices.
- Perform wound treatments, debridement (within scope), dressing changes, and preventive skin care interventions.
- Document assessments, treatments, and progress notes accurately and timely in the EMR.
- Lead weekly or biweekly wound rounds with the interdisciplinary care team.
- Educate nursing staff on wound prevention, skin integrity protocols, and proper dressing/treatment applications.
- Monitor facility-wide skin integrity metrics and collaborate with leadership on quality improvement initiatives.
- Ensure compliance with federal, state, and facility policies including CMS regulations and survey readiness.
- Participate in resident care conferences and provide clinical expertise on wound healing progress.
- Serve as a resource and mentor to nursing staff on all skin and wound‑related issues.
- Current and active RN license in Ohio
- Minimum 1–2 years of nursing experience (long‑term care experience strongly preferred).
- Wound care certification (WCC, WOCN, CWCA, etc.) preferred or willingness to obtain.
- Strong clinical assessment, critical thinking, and time‑management skills.
- Ability to collaborate effectively with residents, families, and multidisciplinary teams.
- Familiarity with EMR systems and regulatory documentation requirements.
