Title: RN Case Manager – Home Health
Status: 6-Month Contract
Location: Cuyahoga County, Ohio
Travel: Mileage Reimbursement at Standard IRS Rate
Sponsorship: No visa sponsor ships are supported
The RN Case Manager will be responsible for providing direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups.
Essential Responsibilities:
Utilizes principles of the nursing process to assist in data collection, planning, and evaluation of nursing care.
Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care.
Utilizes effective interviewing techniques; sets limits, and collects data based on specific roles.
Communicates and collaborates effectively with health care providers, third parties, patients, and families based on specific roles.
Maintains awareness of the comfort and safety needs of patients.
Observe patients, record significant conditions and reactions, and notify the supervisor or Physician of the patient’s condition and reaction to drugs, treatments, and significant incidents.
Responds to life-saving situations based on nursing standards, policies, procedures, and protocols.
Uses oral and written communications to convey pertinent information to members of the healthcare team promptly.
Job Qualifications:
Experience
- 1+ years of experience in Home Health, Emergency Department, CCU, ICU, Med Surg, or similar department
Education
- Associate’s Degree in Nursing or higher
Licence, Certification, Registration
Registered Nurse, licensed in the State of Ohio
Basic Life Support
Valid Driver’s Licence
Additional Qualifications:
Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requires significant collaboration.
Strong planning and organisational skills.
Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
Ability to work independently with minimal supervision.
Demonstrated ability to maintain strict confidentiality.
Bi-lingual a plus.
Job Types: Full-time, Contract
Pay: $34.63 – $43.00 per hour
