We are seeking a compassionate and skilled Hospice LPN Licensed Practical Nurse Scheduler to join our team at Affinity Care of NJ in Parsippany-Troy Hills, New Jersey. As a Hospice LPN Licensed Practical Nurse Scheduler, you will play a critical role in ensuring the smooth operation of our hospice services.
As a Hospice LPN Licensed Practical Nurse Scheduler at Affinity Care of NJ, you will have the opportunity to make a meaningful difference in the lives of terminally ill patients and their loved ones.
- Schedule and coordinate patient visits with our interdisciplinary hospice team, ensuring that patients receive the highest level of care and attention
- Conduct patient assessments and develop individualized care plans in collaboration with the hospice team
- Provide physical, emotional, and spiritual support to patients and their families, promoting comfort, dignity, and quality of life
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Maintain accurate and detailed records of patient care and interactions
- Collaborate with the hospice team to identify and address patient needs, providing education and support as needed
- Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
- As a Hospice LPN Licensed Practical Nurse Scheduler, you will be an integral part of our hospice team, working closely with patients, families, and healthcare providers to deliver comprehensive care and support.
- Licensed Practical Nurse (LPN) certification in the state of New Jersey
- 1 year of experience in hospice care or a related field
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
