* Position Overview:
** Reporting to the Integrated Stroke Unit Manager, the successful Registered Practical Nurse candidate will be responsible for assessing, planning, implementing and evaluating nursing care and practice activities in a multidisciplinary person centered care environment. You will provide acute and rehabilitative care for patients following an acute stroke.
** Requirements:
*** Current certification with the College of Nurses of Ontario in good standing
* Competent to perform CPR at a Basic Cardiac Life Support level and evidence of continued education
* Demonstrated ability to carry out relevant clinical Best Practices, Knowledge integration, and skills within the management of complex stroke cases that bridge the full continuum of care
* Evidence of sound organizational and prioritization skills with the ability to work well in stressful situations
* Demonstrated experience and proficiency in the provision of effective clinical consultation and care for the complex frail elderly
* Excellent communication, documentation, and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community
* Facilitates application of relevant information to make effective judgments and decisions to lead the management of care
* Ability to work well independently, exercise sound judgement, proven analytical thinking and proactive problem solving and decision making skills
* Ability to perform the essential duties of the position
* Demonstrated ability to work well collaboratively in a multidisciplinary team
* Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
* Utilize protective equipment provided and identify and reports hazards
* Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
* Strong work ethic and commitment is required

** Asset:
*** Graduate of an RPN diploma program
* CNA Rehabilitation nursing
* Neuroscience/Stroke Care Courses
* Health Assessment Course or equivalent experience
* Current experience in acute adult medicine and rehabilitation
* Current experience in Stroke/Neuroscience Nursing
