Licensed Practical Nurse

Sevita is seeking a compassionate Licensed Practical Nurse to provide person-centered care in our Healthcare & Medical Services programs. The LPN will deliver direct nursing care, administer medications and treatments, monitor patient conditions, and document in electronic health records. You'll collaborate with an interdisciplinary team, educate individuals and families, and support care plans that promote independence and quality of life. Responsibilities

• Provide direct nursing care in home and community-based settings
• Administer medications and treatments per provider orders
• Monitor, document, and report changes in patient condition
• Maintain accurate, timely electronic health records
• Participate in developing and implementing individualized care plans
• Educate individuals and families on health management and medications
• Collaborate with nurses, physicians, and interdisciplinary team members
• Ensure compliance with infection control and safety standards
• Support person-centered, independence-focused care
• Participate in ongoing training and quality improvement activities
