The Californian - GEM Transition Care Center is a 75 bed skilled nursing facility specializing in short-term therapy and dedicated to providing all of our residents with the type of care they deserve.

We offer a variety of therapies and care services to assist patients in regaining their independence, whether they are recovering from a hospital stay or require ongoing medical support. As the Physical Therapy Assistant and a valued team member, you will collaborate with therapists, patients, their families and the IDT to guide each patient through a personalized therapy program, helping them reach their recovery goals.

REQUIREMENTS
  • CPR Certification (mandatory)
  • Licensed Physical Therapy Assistant (required)
  • Preferably 1-2 years of experience in long-term care.
  • Good physical and mental health are required.
  • Capable of meeting physical demands of the job, including lifting up to 40 pounds unassisted.
  • Proficient in following oral and written instructions.
  • Possess training, knowledge, and experience for general supervision of responsibilities.
  • REPORTING

    The Physical Therapist Assistant reports to the Rehabilitation Director.

    JOB DUTIES
    • Fundamental understanding of physical medicine, physical therapy modalities, anatomy, and physiology.
    • Basic knowledge of operating, maintaining, and repairing physical therapy machines.
    • Administers treatment and rehabilitation services to patients with injuries or disabilities. Conducts exercises with residents.
    • Follows treatment instructions given by the supervising Physical Therapist. Adheres to physical medicine practice standards.
    • Evaluates treatment effects, discusses observations with staff and Physical Therapist.
    • Monitors and documents resident treatment and responses.
    • Contributes to creating a teaching plan based on identified learning needs, assesses teaching effectiveness, involves family when necessary, and informs staff and Physical Therapist of outcomes.
