Clinical Nurse Specialist, Healthcare Nursing
The Coordinator/Navigator position is a Registered Nurse with recognized leadership abilities and sound clinical skills who will actively assist the physician in the day-to-day activities and care of the patient. This role facilitates appointments with specialists and support services, provides patient and site-specific education, and maintains communication with medical, clinical, and support staff as well as the patient and family. The coordinator works with surgeons, oncologists, hospital departments, and out‑patient services to schedule diagnostic and treatment procedures, physician appointments, and supportive care.
They coordinate multidisciplinary treatment conferences and support local oncology teams in delivering high-quality, patient-centered care.
- Provide education using expert oncology knowledge, understanding of insurance coverage, authorization procedures, and community resources. Develop and deliver education plans; delegate reinforcement to other team members as appropriate (e.g., LVN, MA, PCT).
- Coordinate care by assessing clinical conditions, formulating patient-centered care plans, managing complex systems, prioritizing tasks, and coordinating clinical and community services. Participate in clinic visits and ensure all documentation, reports, images, and medication reconciliation are complete.
- Communicate effectively, demonstrating excellent listening skills, strong verbal and written communication, and presentation skills.
- Perform navigation case finding by screening new referrals, contacting patients within 24 hours, following up on tumor board results, and selecting patients for navigation based on diagnosis status, comorbidities, social factors, or referrals.
- Assist with care transitions, develop treatment summaries and survivorship care plans, and support transitions between care phases.
- Manage program data by collecting metrics and creating performance reports.
- Act professionally, building collaborative relationships with patients, teams, and community partners, and applying critical thinking, compassion, empathy, and sensitivity to diversity.
- Participate in community outreach to promote cancer detection and prevention.
- Perform other duties as assigned.
- Bachelor’s degree in Nursing.
- Minimum of 2 years of clinical experience.
- Excellent English communication skills (speaking, reading, writing).
- Knowledge of medical procedures and ability to perform duties such as whole blood collection and apheresis procedures in line with policies and regulations.
- Proficient with computers and Microsoft Office Suite (Word, Excel, PowerPoint).
- Master’s degree in Nursing (completed or in progress).
- Prior experience as a Nurse Navigator or in oncology (minimum 2 years).
Licenses/Certifications:
- Registered Nurse – RN (California DCA).
- Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
- Fire Life Safety Training (LA City)—must be obtained within 30 days of hire and maintained by renewal.
- Chemotherapy and Biotherapy provider card—must be obtained within 6 months of hire (
