Competitive piece‑rate pay with multiple bonus opportunities:

• Per Visit: $40.00 – $45.00 per completed visit (approximate; varies by visit type)
• Mileage Reimbursement: $0.70 per mile
• Drive Time: $25.00 per hour
• Productivity Bonus:

Job Summary
The Licensed Vocational Nurse (LVN) provides high‑quality patient care in the home setting by following the established plan of care under the supervision of the Registered Nurse. This role is ideal for LVNs who enjoy one‑on‑one care, patient education, and helping individuals recover after orthopedic and surgical procedures.

Key Responsibilities

• Deliver nursing care as directed by the RN and patient plan of care.
• Provide accurate, timely, and compliant documentation.
• Collect assessment data and communicate findings to RN and physicians.
• Collaborate with the interdisciplinary care team.
• Use clinical equipment safely and efficiently.
• Engage in ongoing professional development.

Requirements

• Active California LVN license.
• Current CPR certification.
• 2+ years of direct patient care experience (home health experience required).
• Valid driver’s license, reliable insured vehicle, and good driving record.
