Job Description:

• Provides nursing services, treatments and diagnostic and preventive procedures as assigned.
• Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Maintains accurate and complete records of observations, treatments and care of patient.
• Prepares clinical and progress notes.
• Assists the physician and RN in performing specialized procedures.
• Prepares equipment and materials for treatments.
• Observes aseptic technique as required.
• Assists the patient/family in learning appropriate self-care techniques.

• Assists the patient/family in learning appropriate self-care techniques.
• Initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.
• Care Coordinator must develop interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers.
• Must evaluate outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team; implementing team recommendations.
• Be able to communicate with patient and family members, different members of interdisciplinary team, pharmacies, DME company, and more.

• Proven working experience in care coordination, including as a nurse or medical health care manager or a related job.
• Excellent knowledge of case management principles.
• Effective communication skills.
• Compassionate with teamwork skills.

Job Types: Full-time, Part-time

Pay: $35.00 - $40.00 per hour
