Only Care Home Health is looking for a passionate and experienced Director of Nursing to lead our home health clinical team in Pasadena, CA. • Oversee all clinical operations and nursing staff within the home health agency
• Ensure compliance with federal, state, and local home health regulations and Medicare/Medicaid standards
• Develop, implement, and evaluate clinical policies, procedures, and quality improvement initiatives
• Supervise, mentor, and conduct performance evaluations for RNs, LVNs, and home health aides
• Collaborate with physicians and interdisciplinary care teams to coordinate patient care plans
• Monitor patient outcomes and lead QAPI (Quality Assurance and Performance Improvement) programs
• Manage staffing, scheduling, and clinical resource allocation to meet patient care demands
• Serve as a liaison between clinical staff, administration, and external regulatory bodies

• Active RN license in the state of California (required)
• Minimum 2–3 years of home health experience in a clinical supervisory or management role
• In-depth knowledge of Medicare/Medicaid home health Conditions of Participation (CoPs)
• Strong leadership, communication, and team-building skills
• Experience with OASIS documentation and home health EMR systems
• Bachelor's degree in Nursing (BSN) required;
  Master’s degree preferred
• Proven track record in quality assurance, compliance, and clinical program development
• Valid California driver's license and reliable transportation
