Licensed Practical Nurse (LPN) - Outpatient

Licensed Vocational Nurse (LVN) – Outpatient Clinic

Requirements:

• Active California LVN license
• Valid California IV certification
• BLS certification (AHA or ARC only)
• IV & blood draw/phlebotomy certification
• Minimum of 2 years of experience in an outpatient clinic setting
• Experience with EPIC charting system

Roles & Responsibilities:

• Perform patient assessments and document vital signs
• Initiate and manage IV therapy as needed
• Conduct blood draws and lab tests
• Administer medications and vaccines
• Provide phone triage for patient inquiries
• Assist with outpatient procedures within scope of practice

Compliance / Notes:

• Orientation: 40 hours
• Pre-approved time off:
  One period allowed
• Maximum return-to-office/time-off: 7 days; no time off within the first 2 weeks of assignment
• Two supervisor-level references required at time of submission
