Psych Pros is a nationally recognized staffing and recruiting firm dedicated to the behavioral healthcare field. We are currently hiring a Housing Case Manager to work full-time with clients experiencing homelessness in Altadena, CA. This is a TEMPORARY ASSIGNMENT with no set end date.
Requirements- 2 years of experience in the human services field OR a bachelor’s degree in Social Work or in a related field
- Possess a valid California driver's license and have access to properly registered vehicle
- $23.50/hour
- Monday-Friday 8:30am-5pm
- Provide intensive case management to clients living in a permanent supportive housing program who also have a chronic health condition
- Conduct intake and enrollment with eligible clients
- Assist clients with accessing temporary housing until permanent housing is secured
- Support clients with the lease‑up process
- Conduct comprehensive bio‑psycho‑social assessments
- Develop individualized collaborative service plans
- Assist clients with their physical and mental health needs by providing support and linkage to appropriate services.
- Transport clients as needed to essential appointments
