Evolving Solution Services is seeking a Home Health Physical Therapist for PRN assignments across designated counties in Texas. You will evaluate patients, design individualized rehab plans, and apply manual therapy and therapeutic exercises to restore mobility and function.
As part of a multidisciplinary team, you will educate patients and families, monitor progress, and adapt treatment to achieve meaningful improvements in daily living activities. Licensure and DPT are required.
