The Harris Center for Mental Health and IDD is seeking a Care Coordinator to provide case management and care coordination in community housing programs. The role focuses on engaging with individuals, coordinating services, and supporting overall housing and mental health goals.
The position requires a 4-year degree in a social/behavioral field and a Texas driver’s license. You will work with a multidisciplinary team, including a LPHA and LCDC, and may provide transportation for clients.
