Behavioral Health Rep II - Job Overview

Schedule:
Monday-Friday (8AM-4:30PM) | Hybrid

Bring your behavioral health or clinical experience to a role where your compassion, organization, and follow-through help patients receive timely, coordinated care. As a Bilingual Behavioral Health Representative II, you will work closely with on-site Behavioral Health providers—especially Psychiatrists—to support patient rooming, referrals, risk assessments, care management, and thoughtful communication between patients and providers.

You will join a collaborative clinic team where every interaction can make a difference in a patient’s behavioral health journey. Along with supporting patients and providers, you will assist the Behavioral Health Supervisor, contribute to training and one‑on‑one shadowing, and help strengthen access to essential community healthcare services.

  • Contribute to accurate records, timely information gathering, and coordinated care across the healthcare system.

Your Work Environment
  • Collaborate in a clinic-based setting where communication, teamwork, and mutual support are essential.
  • Cross-cover remotely and travel to other locations for coverage as needed.
  • Balance direct patient support, care coordination, administrative responsibilities, and team development.
  • Make a meaningful contribution to patient access and the broader community healthcare network.
Key Responsibilities
  • Provide support to on-site Behavioral Health providers, with a focus on Psychiatrists.
  • Take patient vitals and complete patient rooming.
  • Manage referrals to providers.
  • Screen new and existing patients using risk assessments.
  • Take responsibility for patient care management.
  • Communicate by phone with providers and patients.
  • Assist the Behavioral Health Supervisor with duties as assigned.
  • Perform Behavioral Health Representative II duties in addition to all Behavioral Health Representative I duties and provide coverage as needed.
  • Manage efforts surrounding BI reporting.
  • Obtain collateral patient information from schools and other providers or hospitals as needed.
  • Process external referrals in a timely manner.
  • Ensure all referrals are completed and relevant documents are included in the medical record, including documentation related to Neurology, speech therapy, cardiology, and other services.
  • Review the schedule daily to ensure all pre‑visit preparations have been completed before the patient’s visit.
  • Ensure lab and imaging results, consultation reports, care transition details, and significant changes in health status are available in the patient’s medical record.
