Overview

Focused Post Acute Care Partners FPACP is seeking a Charge Nurse to join our team. The position provides nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices, regulations, and the direction of the Director of Nursing. The role includes leading or directing licensed and non-professional staff in the delivery of direct resident care and support functions.

• Current LVN/RN license
• CPR certification required
• Ability to understand, remember and carry out verbal or written instructions in English
• Ability to complete assignments in a timely manner
• Ability to be flexible and positive with intermittent interruptions

• Plan, organize, and direct the nursing functions of patients in the unit
• Make nursing assignments appropriate to the skill level of employees
• Maintain acceptable standards of patient care
• Identify problems and guide personnel to solutions
• Create a working climate that provides growth and job satisfaction
• Accurately and promptly implement physicians orders
• Support and enforce infection-control policies and procedures
• Maintain a professional approach with confidentiality
• Cooperate and maintain good rapport with nursing staff, medical staff, other departments, and visitors
• Obtain and document nursing history from the patient and/or significant other
• Perform basic assessment of the patient with knowledge of anatomy, physiology, and pathology
• Record vital signs, note changes, and pursue further investigation as needed
• Assess for and notify physician of changes in condition
• Develop a care plan before the end of duty time for incoming personnel
• Use care plans to provide safe and therapeutic care
• Follow established standards, policies, and procedures
• Accept accountability for clinical care of assigned patients, including supervision of nonprofessional personnel
• Communicate with patients, families, and health care team to carry out the care plan goals
• Perform basic technical skills essential to the unit
• Respond appropriately in urgent and/or emergency situations
• Support standards of nursing care through adherence to policies and procedures
• Seek guidance when encountering procedures not yet mastered
• Promote positive public relations with patients, residents, family members, and guests
• Use tactful, appropriate communications in sensitive situations
• Maintain appropriate workplace behavior and dress code
• Utilize electronic medical record charting (Point Click Care )

Ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds with or without reasonable accommodations.
