Work Schedule: 3 days per week, 7p-7a/Rotating Weekends

This frontline leadership position is strategically tailored to attract high-caliber Long-Term Care (LTC), Geriatric, Post-Acute Care, Sub-Acute Care, and SNF Floor RNs who are ready to combine advanced transitional care expertise with shift operations, patient throughput coordination, and staff mentorship.

Operating within our specialized post-acute and rehabilitative care unit, you will oversee comprehensive multi-system medical management for patients stepping down from acute hospital stays. You will guide the delivery of restorative care, complex wound care, IV therapies, advanced enteral nutrition, and personalized [Minimum Data Set (MDS) resident care plans]in close coordination with an interdisciplinary team of physical, occupational, and speech therapists.

Job Summary and Qualifications

The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization’s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.

What you will do in this role:

  • Assist with the admission and discharge processes to ensure efficient unit throughput and the delivery of high-quality, patient-centered care.
  • Manage staff scheduling and all practical aspects of labor productivity in accordance with patient care needs and established guidelines.
  • Partner with management teams and subject matter experts to evaluate patient care services and foster a teamwork-driven clinical environment.
  • Support a patient-first philosophy by operationalizing strategic nursing initiatives, including hourly patient rounds, employee rounding, and executing service recovery.
  • Provide recommendations for interviewing and hiring, while implementing development plans, conflict resolution strategies, performance evaluations, and disciplinary actions.
  • Maintain strict department inventory control by assisting with the active management of clinical supplies and medical equipment..

What qualifications you will need:

  • Graduate of an Accredited School of Nursing is required. BSN is preferred.
  • Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License with 90 days of hire date).
  • Current BLS certification are required.
  • 1-year of Skilled Nursing Facility SNF clinical experience as a licensed RN in an acute care setting required.
