Outpatient Case Management RN

Shift: Monday – Friday, 8:00am – 4:30pm.

Location:

Singing River Pascagoula Hospital.

The Outpatient Case Management Nurse will coordinate the interdisciplinary plan of care for the non‑hospitalized population (multi‑visit patients, TCM, addressing SDOH) within the SRHS service area. The nurse acts as a liaison between the patient/family and healthcare personnel to ensure all necessary care is provided and promotes quality, cost‑effective care and patient outcomes. This role requires creative problem‑solving and independent judgment to plan and accomplish goals and perform a variety of tasks.

• Graduate of an accredited School of Nursing.
• Currently licensed to practice as a nurse in the State of Mississippi with no encumbrances.

• Required:
  
  Two or more years of direct patient care or clinic experience.
• Preferred:
  Experience in case management.

Director of Case Management.

Work is moderately active, requiring sitting with frequent movement, transporting patients or materials within the facility, and traveling to other sites in the SRHS service area. The role involves occasional lifting, carrying, pushing, pulling, and repetitive wrist and hand motions while using standard office equipment. Tasks may expose staff to bodily fluids and other infectious hazards. Visual acuity, fine discrimination of sound, and sustained physical activity without undue fatigue are required.

The role requires strong critical thinking, decision‑making, communication, and writing skills. The nurse must manage high stress, work under pressure, meet deadlines, and maintain attention to detail while multitasking in complex situations. Emotional stability, effective collaboration, and professional etiquette are essential.

Proficient use of computers and office technology (Microsoft Outlook, Word, fax, telephone) is required. The nurse must demonstrate clinical judgment and provide primary care to patients of all ages.
