Position Overview
Location:
Singing River Pascagoula Hospital
Shift: Monday - Friday (8:00 AM - 4:30 PM)
The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System and ensure all duties comply with facility procedures, policies, accreditation standards, and governing guidance. The role requires working effectively under pressure, multitasking, and being self‑directed.
Responsibilities- Participate in daily discharge planning meetings, addressing barriers to discharge and promoting successful transitions.
- Maintain seamless communication between inpatient care teams and outpatient case management.
- Coordinate and manage patient care plans across the organization.
Graduate of an accredited School of Nursing;
Bachelor’s Degree in Nursing preferred.
Licensed to practice as a Registered Nurse in the state of Mississippi.
CertificationBCLS certification upon hire and maintenance; TB certification preferred. De‑escalation training completed within 90 days of orientation.
ExperienceMinimum 2 years of nursing experience in ambulatory or acute care relevant to case management.
Physical DemandsModerately active work involving sitting, moving around the facility, and occasional travel. Requires occasional light lifting and frequent use of repetitive motions (e.g., keyboard, monitor). Exposure to body fluids and potential infectious hazards. Vision and hearing within normal ranges required. Ability to work scheduled shifts, including occasional changes.
Mental DemandsHigh level of decision‑making, assessment, and communication skills. Ability to manage stress, multitask, meet deadlines, and maintain attention to detail.
Special DemandsExcellent customer‑service skills, professional etiquette, and proficiency with computer and office technology (MS Outlook, Word, phone, fax). Strong clinical judgment and ability to care for patients of all ages.
