Optum Home & Community Care (Senior Community Care) is hiring a Per Diem Nurse Practitioner or Physician Assistant to provide high-quality, coordinated care to Medicare and Medicaid patients in nursing homes, assisted living, senior housing, and occasional private residences
.
In this role, you’ll deliver primary care services
, manage chronic and acute conditions
, support care transitions, and partner closely with facility staff, families, and physicians to improve outcomes and reduce unnecessary hospitalizations.
- Care delivered in home and facility-based settings
- Strong focus on geriatrics, chronic illness, and care coordination
- Travel within assigned area required
- NP or PA board certified
- DEA (or ability to obtain)
- 1+ year post-grad experience preferred (LTC experience a plus)
