Responsibilities- In-home patient assessments
- Individualized care plan development
- Periodic supervisory visits
- Family communication
- Clinical on-call support
Flexible scheduling — full-time and per-diem positions available.
Requirements- Active New Jersey RN license required.
- Home care, hospice, or community health experience strongly preferred.
- Must have reliable transportation for in-home supervisory visits.
