Job Description
Registered Nurse responsible for working with other healthcare professionals to treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keeps records, consults healthcare providers, educates patients and more. Assesses patient knowledge and provides education in group and individual settings for patients with Type 1, Type 2, Gestational Diabetes, and Pre‑Diabetes. Provides education tailored to special learning needs. Coordinates patient care and adjustments in care plans in coordination with clinicians.
PrincipalAccountabilities
- Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.
- Collaborates with the referring clinicians and interdisciplinary team to work toward optimal patient education outcomes and glycemic control.
- Maintains professional education standards required to be a CDCES in an ADA approved Diabetes Self-Management Education program.
- Engages in research and quality improvement activities.
- Other duties as assigned.
- Per diem, Evenings
- Monday – Friday (2 shifts per month)
- Schedule may vary based on department needs
- RN education obtained from an accredited institution
- BLS required
- Certification as a CDCES (Certified Diabetes Care and Education Specialist) requested within 2 years of start date
- Prior RN roles in acute care or ambulatory diabetes
- Minimum of 1–3 years RN experience
- Epic experience preferred
- BSN preferred
- Medical, Dental, Vision, Prescription Coverage (22.5 hours per week or above for full‑time and part‑time team members)
