Job Summary

Ennoble Care is hiring a full‑time Hospice Registered Nurse Case Manager for the Gloucester / Camden County, NJ region. The RN Case Manager works under the direction of the Director of Nursing, the attending physician, and the interdisciplinary team to provide skilled nursing assessments, planning, and care that maximize comfort and health for patients and their loved ones or caregivers.

Responsibilities
  • Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process.
  • Initiate communication with attending physicians, Hospice staff, and other agencies as needed to coordinate optimal care and use of resources.
  • Maintain regular communication with the attending physician concerning patient and family needs.
  • Seek input from team members regarding the patient plan of care to obtain additional knowledge and support.
  • Provide accurate and timely documentation in electronic medical records.
  • Participate in and contribute appropriate information for Interdisciplinary Group meetings.
  • Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, or providing direct care as appropriate.
  • Meet regularly with Hospice nursing staff to review problems or unique issues from caseloads; share professional support and exchange feedback to enhance professional growth.
  • Attend patient care conferences and facilitate discussion of issues for full staff participation, consultation and evaluation.
  • Supervise and document the NAC every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues.
  • Assume the role of associate nurse during on‑call periods or when other primary nurses are absent.
  • Provide appropriate support at the time of death and during bereavement.
  • Participate in orientation and in‑service training programs.
  • Participate in agency and community programs to promote professional growth and understanding of hospice care.
  • Maintain familiarity with agency policies and state/federal regulations governing hospice.
  • Accept other assignments as appropriate.
Qualifications
  • Graduate from an accredited School of Nursing.
  • Licensed in NJ as a Registered Nurse by the New Jersey State Board of Nursing in accordance with NJSA 45:11‑23 and NJAC 13:37.
  • One year of RN experience; hospice experience preferred.
  • Excellent assessment and problem‑solving skills.
  • Strong organizational skills, time management principles, and knowledge of case management processes.
  • Licensed driver with an insured automobile in good working order.
  • Strong verbal and written communication skills.
  • Fluent in both English and Spanish.

Starting at $85,250.
