We are looking for an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in the Ocean County, NJ region. The NP will apply medical techniques under physician supervision and care for a diverse set of patients with chronic diseases.
Responsibilities- Deliver high quality primary and palliative care to patients in and around Ocean City, NJ.
- Assess patients’ health through physical examinations, medical history review, and develop care plans.
- Communicate effectively with office staff and clinical operations across internal platforms.
- Provide patient‑centered care to a panel of patients.
- Attend quarterly medical staff meetings.
- Develop impressions, diagnoses, and treatment plans.
- Foster trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary.
- Prescribe medications, supplies, and other medically necessary interventions.
- Board Certified with a valid NP or PA certification in New Jersey.
- Strong clinical skills with the ability to assess, diagnose, and treat confidently.
- Positive demeanor and excellent people skills.
- Capable of problem‑solving and operating independently.
- 1–2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required).
- Flexible schedule to accommodate personal needs.
- Autonomy to provide the best care possible.
