Job Description

Atlantic Health is seeking a Part‑Time Nurse Practitioner (NP) to join the Assisted Living and Home Visit Team near Union, New Jersey. The NP will visit patients aged 65+ in their homes and assisted‑living facilities for primary care visits in the Union County area.

• Provide primary care visits to patients 65 years or older in their homes and assisted‑living facilities.
• Coordinate care with resident physicians and support staff.
• Follow established protocols for geriatric and internal medicine care.
• Maintain accurate patient records and documentation.

• Master's degree.
• Nurse practitioner from an accredited program.
• Licensed or eligible for licensure in New Jersey.
• ACLS and BLS certification required.
• Minimum of 5 years of experience as a practicing NP in internal or geriatric medicine.
• Strong work ethic, teamwork, and ability to work independently.

Part‑time, 15 hours per week with no weekend requirement or call duties.
