Affirmed Home Care is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to deliver exceptional in-home healthcare services throughout Northern and Central New Jersey.
We are hiring immediately for nurses who are passionate about enhancing the well-being, dignity, and independence of clients in the comfort of their own homes. Compensation & Benefits- Competitive Pay: $40–$45 per hour, plus performance-based incentives

- Provide skilled nursing care under the supervision of an RN and in accordance with care plans
- Administer medications, treatments, and wound care as prescribed
- Monitor and document patients’ conditions, reporting changes promptly to the RN or supervisor
- Assist with IV therapy, catheter care, and other skilled nursing tasks as permitted by scope
- Educate clients and families on medication adherence and home safety practices
- Maintain accurate, timely, and compliant documentation in EHR systems
- Promote a safe, respectful, and supportive environment for clients and families
- Active LPN License in the State of New Jersey (required)
- 1–2 years of clinical experience preferred (home care, long-term care, or related field)
- Excellent communication, assessment, and time-management skills
- Compassionate, dependable, and able to work independently in a home care setting
- CPR/BLS certification (required)
- Valid driver’s license and reliable transportation
- Experience with Electronic Health Records (EHR) preferred
