Eagle Rock Post Acute Care at Cedar Grove seeks a compassionate LPN to provide resident-focused care in our skilled nursing and post-acute rehab center. Responsibilities include medication administration, treatments, monitoring changes in condition, and accurate documentation in collaboration with RNs, therapists, and physicians. You will support both short-term rehab and long-term residents, participate in care planning, and communicate closely with families.
