Overview

Swan Hospice is actively seeking Registered Hospice Nurses (RN) for full-time or part-time roles in New Jersey. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities, and private homes.

Job Description

  • BASIC

    PURPOSE:

    Oversees a primary caseload of hospice patients and provides skilled professional nursing care to patients, families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey hospice licensure laws, accrediting body standards, and agency policies and procedures. Conducts initial and comprehensive nursing assessments, identifies patient problems, and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care.

    Implements and reevaluates the plan of care relative to the patient’s nursing needs to achieve appropriate, comprehensive goals and interventions. Teaches, supervises, and counsels patients, families, and staff regarding nursing care and patient needs, including related home-care issues. Works closely with IDT members to ensure all patient and family care needs are met. Provides supervision to Hospice Aides/Homemakers.
  • In the temporary absence or vacancy of the Nursing Supervisor, the RN/Alternate Nursing Supervisor is authorized to assume the role of Nursing Supervisor and perform the duties outlined in the Nursing Supervisor’s Job Description (copy attached).
Responsibilities
  • PRIMARY RESPONSIBILITIES
  • Nursing Process – As a member of the IDT, the RN utilizes the nursing process to contribute to each patient’s total plan of care.
  • Assessing and Planning
    • Seek and obtain sufficient information to function effectively in the Hospice setting where the patient/family/caregiver is receiving care.
    • Conduct initial assessments (physical, emotional, psychosocial) of patient/family/caregiver in a thorough, accurate, compassionate, and timely fashion.
    • Utilize the nursing body of knowledge to lead IDT in formulating and individualizing the interdisciplinary plan of care, including problem identification, interventions, and goals for palliative care.
    • Identify active and potential needs of the patient and family that may affect the plan of care.
    • Collaborate with the physician to plan and implement the medical and nursing plan of care.
    • Review the nursing component of the interdisciplinary plan of care with the IDT and communicate in writing in the clinical record and verbally with the IDT to coordinate interventions and ensure continuity of care.
    • Facilitate and participate in IDT care conferences and contribute to patient care planning.
  • Implementing and Evaluating
    • Provide nursing interventions and appropriate preventive and therapeutic nursing procedures.
    • Administer medications in accordance with all Federal and State laws and rules.
    • Collaborate with IDT members to continuously assess assigned patients, interpret findings, initiate appropriate actions, and evaluate results.
    • Perform specialized procedures only when qualified and competent in the specialty area.
    • Work with family/caregiver and IDT to meet physical and emotional needs of the patient/family/caregiver.
    • Provide information and explanations to the patient and family.
    • Facilitate patient and family education to promote continuity of care and quality of life as defined by patient/family/caregiver.
    • Demonstrate expertise in utilizing all IDT members, including volunteers and community resources.
    • Fulfill productivity requirements on an average daily census basis as determined by acuity level and Administrator.
    • Document accurate, complete, and timely clinical notes regarding patient condition and care.
    • Assess dietary/nutritional needs and provide appropriate interventions relative to nutritional intake as the patient’s condition progresses.
Education

The RN is a mentor who provides educational experiences and communicates knowledge of patient care to IDT members, volunteers, hospice aides/homemakers, patients and families, health care professionals, and the community. Acts as a mentor during orientation and field training, and supports in-service education for contractors and referral sources as needed.
