The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Responsibilities- Advocate for the patient and family throughout the entire episode of care.
- Monitor patient progress, intervening as needed to ensure efficient patient progress.
- Coordinate all aspects of discharge planning and establish a safe discharge plan.
- Required
- BSN
- 2 years of medical‑surgical nursing experience.
- Preferred
- 1–2 years of acute care, home care or population health RN case‑management experience.
- CCM certification.
- Monday‑Friday 8:30‑3:30
- One weekend day shift every 4 weeks.
- One major holiday per year.
- Willingness to occasionally float to all three campuses.

Salary Range$37.65 to $53.67 per hour (actual rate determined by experience, skills and qualifications). Not eligible for an annual incentive.
